Goals/Objectives:
To assist program participants in stabilizing their health.
Target:
Urban aboriginal persons of all ages that have physical disabilities, serious health issues or the frail/elderly and chronically ill.
Services Provided:
- Assistance to coordinate medical appointments
- Home visits
- Congregate dining
- Physical fitness
- Cultural programming
- Social activities
- Referrals and advocacy
- Access to community supports
- Transportation
